Orsini Pharmaceutical Services (“Orsini”) is committed to protecting the confidentiality and security of the information
we maintain. We are writing to let you know about a data security incident that may have resulted in unauthorized access
to some of your information. This letter explains the incident, measures that have been taken, and some steps you can take
in response to protect your information.
On January 10, 2024, we were alerted to unusual activity in a single employee’s email account. Upon learning of this, we
immediately took steps to secure the email account and launch an investigation. Through our investigation, we confirmed
that the scope of the incident was limited to a single employee’s email account and did not involve unauthorized access to
any other Orsini systems. We further determined that an unauthorized party gained access to the single employee’s email
account between the dates of January 8, 2024 and January 10, 2024. Although we did not find evidence suggesting that
the purpose of the unauthorized access to was to obtain patient information, we could not rule out the possibility that such
information may have been accessed and/or acquired as a result of this incident. Thus, out of an abundance of caution, we
analyzed emails and attachments in the email account to determine their contents.
Through this analysis, we identified emails that contain some of your information, including your name and Social Security
number, and may have included one or more of the following: address, date of birth, medical record number, health
insurance information, diagnosis, and/or prescription information.
As a precaution, we are offering you a complimentary one-year membership to Equifax Complete Premier, which includes
credit monitoring and fraud alerts. For more information on identity theft prevention and Equifax Complete Premier,
including instructions on how to activate your complimentary membership, please see the pages following this letter.
We deeply regret any inconvenience or concern this may cause and take this matter seriously. To help prevent something
like this from happening again, we have implemented, and will continue to adopt, additional safeguards and technical
security measures to further protect and monitor our systems. If you have any questions about this incident, please call

Enter your unique Activation Code of <<ACTIVATION CODE>> then click “Submit” and follow these 4 steps:
1. Register:
Complete the form with your contact information and click “Continue”.
If you already have a myEquifax account, click the ‘Sign in here’ link under the “Let’s get started” header.
Once you have successfully signed in, you will skip to the Checkout Page in Step 4
2. Create Account:
Enter your email address, create a password, and accept the terms of use.
3. Verify Identity:
To enroll in your product, we will ask you to complete our identity verification process.
4. Checkout:

The credit scores provided are based on the VantageScore® 3.0 model. For three-bureau VantageScore credit scores, data from Equifax®, Experian®, and TransUnion® are used
respectively. Any one-bureau VantageScore uses Equifax data. Third parties use many different types of credit scores and are likely to use a different type of credit score to assess your
creditworthiness.

Credit monitoring from Experian and TransUnion will take several days to begin.

WebScan searches for your Social Security Number, up to 5 passport numbers, up to 6
bank account numbers, up to 6 credit/debit card numbers, up to 6 email addresses, and up to 10 medical ID numbers. WebScan searches thousands of Internet sites where consumers' personal
information is suspected of being bought and sold, and regularly adds new sites to the list of those it searches. However, the Internet addresses of these suspected Internet trading sites are not
published and frequently change, so there is no guarantee that we are able to locate and search every possible Internet site where consumers' personal information is at risk of being traded.

Locking your
Equifax credit report will prevent access to it by certain third parties. Locking your Equifax credit report will not prevent access to your credit report at any other credit reporting agency.
Entities that may still have access to your Equifax credit report include: companies like Equifax Global Consumer Solutions, which provide you with access to your credit report or credit
score, or monitor your credit report as part of a subscription or similar service; companies that provide you with a copy of your credit report or credit score, upon your request; federal, state
and local government agencies and courts in certain circumstances; companies using the information in connection with the underwriting of insurance, or for employment, tenant or
background screening purposes; companies that have a current account or relationship with you, and collection agencies acting on behalf of those whom you owe; companies that authenticate
a consumer's identity for purposes other than granting credit, or for investigating or preventing actual or potential fraud; and companies that wish to make pre-approved offers of credit or

We remind you it is always advisable to be vigilant for incidents of fraud or identity theft by reviewing your account
statements and free credit reports for any unauthorized activity. You may obtain a copy of your credit report, free of charge,

If you believe you are the victim of identity theft or have reason to believe your personal information has been misused,
you should immediately contact the Federal Trade Commission and/or the Attorney General’s office in your state. You can
obtain information from these sources about steps an individual can take to avoid identity theft as well as information about
fraud alerts and security freezes. You should also contact your local law enforcement authorities and file a police report.
Obtain a copy of the police report in case you are asked to provide copies to creditors to correct your records. Contact

Fraud Alerts: There are two types of general fraud alerts you can place on your credit report to put your creditors on notice
that you may be a victim of fraudan initial alert and an extended alert. You may ask that an initial fraud alert be placed
on your credit report if you suspect you have been, or are about to be, a victim of identity theft. An initial fraud alert stays
on your credit report for one year. You may have an extended alert placed on your credit report if you have already been a
victim of identity theft with the appropriate documentary proof. An extended fraud alert stays on your credit report for seven
years. To place a fraud alert on your credit reports, contact one of the nationwide credit bureaus. A fraud alert is free. The
credit bureau you contact must tell the other two, and all three will place an alert on their versions of your report. For those
in the military who want to protect their credit while deployed, an Active Duty Military Fraud Alert lasts for one year and
can be renewed for the length of your deployment. The credit bureaus will also take you off their marketing lists for pre-
screened credit card offers for two years, unless you ask them not to.
Credit or Security Freezes: You have the right to put a credit freeze, also known as a security freeze, on your credit file,
free of charge, which makes it more difficult for identity thieves to open new accounts in your name. That’s because most
creditors need to see your credit report before they approve a new account. If they can’t see your report, they may not extend
the credit.
How do I place a freeze on my credit reports? There is no fee to place or lift a security freeze. Unlike a fraud alert, you must
separately place a security freeze on your credit file at each credit reporting company. For information and instructions to
place a security freeze, contact each of the credit reporting agencies at the addresses below:

You'll need to supply your name, address, date of birth, Social Security number and other personal information. After
receiving your freeze request, each credit bureau will provide you with a unique PIN (personal identification number) or
password. Keep the PIN or password in a safe place. You will need it if you choose to lift the freeze.
How do I lift a freeze? A freeze remains in place until you ask the credit bureau to temporarily lift it or remove it altogether.
If the request is made online or by phone, a credit bureau must lift a freeze within one hour. If the request is made by mail,
then the bureau must lift the freeze no later than three business days after getting your request.
If you opt for a temporary lift because you are applying for credit or a job, and you can find out which credit bureau the
business will contact for your file, you can save some time by lifting the freeze only at that particular credit bureau.
Otherwise, you need to make the request with all three credit bureaus.
